Basic – Level 1 – Formation Page (2 pages)
Intermediate – Level 2 – Formation Page (2 pages)
Advanced – Level 3 – Formation Page (2 pages)
Professional – Level 4 – Formation Page (2 pages)
Renewal – All Levels (2 pages)
Recommendation for Catechist Certification (2 pages)
Catechetical Leader Record (4 pages)
Catechist Record (4 pages)

All applicants must submit the following:

1. 1. Completed Forms
   a. Application For Catechist Certification
   b. One of the Formation Pages listed below, depending upon the "Level" of certification desired. If renewing certification, the renewal form is used.
   c. Recommendation for Catechist Certification
